General Consent Of Personal Information Form

The NY DMV application can be downloaded HERE. You must complete the DMV application and the General Consent Of Personal Information Form get them notarized and returned with the proper payment to:

You must have at least 3 months left before expiration on your current registration and we must have the original notarized form. No faxes can be accepted.

The NJ DMV form cannot be downloaded from the website and must be ordered by calling the National Police Defense Foundation at 732-617-2330 or requesting it via email. Once you receive it, legibly complete the form and include a copy of your current registration with at least 3 months left before expiration.
